I’m 25, Class II Division 2, and my concern is purely aesthetic—my bite works fine and I have no functional issues.
Here’s where I’m stuck:
Two orthodontists recommended removing two lower premolars
One ortho said I could prep for BSSO in 6–8 months with no extractions
Most surgeons suggest just genioplasty since my bite is already functional
The ortho who suggested extractions refused to prep me for BSSO, saying I don’t need jaw surgery
I also have a strict no-extractions policy.
Main questions:
Is BSSO without extractions realistic for aesthetic improvement?
Is BSSO + genio worth it, or should I just go for genioplasty alone?
If I start braces for decomp, am I basically committing to surgery?
I’ll be uploading my lateral ceph and OPG for context.
Would really appreciate any advice or experiences—especially from people who’ve been in a similar situation.
