What to tell your pcp and job

I am so lost I will be having surgery end of February in DR, What do I tell my job for time off they told me I need to file a claim for disability and then I'm just wondering if my pcp will prescribe my abt for surgery or do I need to buy them in DR I have a appt coming up with my primary I'm just hoping she won't say get your paperwork from your surgeon. How did you go about these things? Thanks
