Have anyone on over 190.lbs had an EXTENDED TT? NEED INFO

After a year of research and consultations with two doctors, it was recommended that I get an extended TT.  I am 55  yo, 210 lbs and had two Caesareans. I have always been between 190-220 lbs.  So my weight is settled.  I am not looking to be skinny by the least but need to get rid of the hanging fold that seems to be tearing at the fold of the skin lately and I just want to get it off!  And would like to get rid of the muffin top under my breast.  Not necessarily needing to got through any additional and unnecessary pain to get tight little waistline. Just want to get rid of the puffy and hanging fat!I was wondering if besides the muscle repair portion, would would it be like if I should just consider getting an upper and lower abs and sides?  Anyone been here?
