Any advice on what to tell my employer?

I work in financial sales (desk job, little to no strain) and have very limited vacation/sick leave. I am not shy about my procedure, and the environment at our office is very fun and friendly, but  I don't feel comfortable advertising it since I work with mostly men. I have already told my boss that I will be having surgery, and he was very understanding. He didn't ask what it was for, but I am sure he's assumed it is a medical procedure instead of a cosmetic one. I am concerned that after the surgery it will be obvious that I have breast implants and my time off will be perceived as voluntary (since the surgery is voluntary) as opposed to medical sick leave. I don't think that I should be held more accountable for plastic surgery than someone who had knee surgery, but this is the real world and people have preconceived notions about this. Does anyone have advice or personal experiences to share that may help me? What did you tell your employer? And how much time did you request off beforehand? Did the fact that it is a cosmetic procedure have any affect on how your boss reacted?
