Face implants. Chin, jaw eyelids upper and lower, nose revision

I need help. 

I m at a point of my life where I don't like what I see in the mirror, I do not take pictures anymore... I've had multiple procedures in the past and I really need a doctor to take care of my concerns and also affordable.

I remember finding here a Dr. In Minnesota (I guess) that will scan your face and print with a 3D printer all the implants, I m looking for jaw and chin and maybe cheeks, also my upper and lower eyelids, CO2 laser, and a nose job revision.. 

Can anyone please help me with suggestions..? I live in NYC but willing to travel since in the city is really expensive.
