How can you tell if you have an infection or a hematoma?

I saw the nurse today who removed my sutures. I had a mini facelift and neck lipo 8 days ago. I have swelling behind my left ear and below the ear lobe. It is red, raised, tender, and spongy. The nurse believes it was caused by the pressure bandage rubbing against the area. I am to continue another round of antibiotic, but she said it might be a small hematoma. Now, that I am thinking this over I am concerned either way. If it is a hematoma that isn't good and if it is an infection, that is a concern. Anyone experience anything like this?
