What types of pain does insurance cover for implant removal? Specifically Kaiser in southern CA, but any info will help.

I have some moderate pain with my left breast, but other than that everything seems normal (non medical opinion). I understand Kaiser may help if there is pain present. I don't want to have the wrong kind of pain. I would really like to be able to use the insurance we spend so much for every year. I have an appointment with the breast care center soon to decide what kind of mammo to get.Thanks in advance for any help you can provide.
